𝗦𝗻𝗮𝗽𝗽𝗶𝗻𝗴 𝗯𝗶𝗰𝗲𝗽𝘀
👉Audible and visible snapping of the biceps femoris tendon over the lateral aspect of the fibular head in deep flexion. The etiology is often not recognized, most present with symptoms of painful snapping knees without a history of trauma. Diagnosis is primarily performed by physical examination, with MRI providing minimal information.
🔹A lateral hockey-stick skin incision is made along the iliotibial band and extended distally, halfway between the fibular head and Gerdy tubercle
🔹Detachment of the injured direct arms of short and long heads is performed
🔹Reattachment with 2 or 3 anchors (here with Y-knot by Conmed)
🔹Postop: no weight bearing for 6 weeks. ROM 0-90 for 2 weeks. Isolated hamstring curls are avoided for 4 months. Return to full activities around 5 to 6 months postop.

👉Chasquido audible y visible del tendón del bíceps femoral sobre la cara lateral de la cabeza del peroné en flexión profunda. La etiología a menudo no se reconoce, presentándose en general con dolor en las rodillas, a veces con síntomas neurológicos por el nervio peroneo, sin antecedentes de traumatismo. El diagnóstico se realiza principalmente mediante examen físico, y la resonancia por lo general es normal.
🔹Se realiza una incisión cutánea lateral en palo de hockey a lo largo de la banda iliotibial y se extiende distalmente, a mitad de camino entre la cabeza del peroné y el tubérculo de Gerdy
🔹Se desinsertan las inserciones directas de las cabezas cortas y largas del bíceps
🔹Se repara con 2 o 3 anclajes en la cabeza del peroné (aquí con anclajes Y-knot de Conmed)
🔹Postop: 6 semanas sin carga. 0-90 durante 2 semanas. Ejercicios de flexión activa se evitan durante los primeros 4 meses. Retorno deportivo a los 5 a 6 meses.

However, the reported failure rates raises up to 40%. 🔹Hamstring grafts in children are usually of small diameter. 🔹Harvesting the tendon from the parents, we decrease the morbidity and maintain the autologous hamstring tendons as a graft source for an eventual revision surgery. 🔹All-inside drilling 🔹Tibial tunnel with retrograde drilling (though it can be done anterogradely transphyseal too) 🔹Femoral tunnel always physeal-sparing. 🔹Always back up the ACL graft with a lateral tenodesis to reduce failure rate (Arnold Coker technique, implant-free). 🔹RTP is never allowed before 10 to 12 months (Longer than in adults) . SE OPERAN SIEMPRE! 🔹Los injertos de isquiotibiales en niños suelen ser de pequeño diámetro. 🔹Con el semitendinoso de los padres, disminuimos la morbilidad y mantenemos los tendones propios como fuente de injerto para una eventual cirugía de revisión. 🔹Técnica all-inside 🔹Túnel tibial con fresado retrógrado (aunque también se puede hacer anterógradamente transfisario) 🔹Túnel femoral siempre respetando la fisis, con fresado retrógrado. 🔹Siempre proteger el injerto de LCA con una tenodesis lateral para reducir la tasa de fracaso (técnica de Arnold Coker, sin implantes). 🔹Nunca se permite retorno deportivo antes de los 10-12 meses (más que en adultos) . ¿Estás de acuerdo con todos estos aspectos? ¿Qué propondrías alternativamente? ¡Deja tu comentario!$
