In cases with extensive chondral patellofemoral lesions and even isolated osteoarthritis of the patellofemoral joint in young patients, we found the alternative of fresh osteochondral transplantation. In this way, we avoid the placement of a patellofemoral prosthesis in patients who do not yet have a chronological or biological age for it.
Fulkerson osteotomy is indicated in people with chronic patellofemoral or anterior knee pain, in whom the TA-GT distance is greater than 15mm. The technique is described here with Dr. Gelber's personal preferences. It could be considered as one of the most gratifying surgeries of knee orthopedics.
